Your GP will assess if you would benefit from an HMR and provide a referral.

A friendly clinical pharmacist will visit you at home to review your medications and discuss any concerns.

The pharmacist will prepare a comprehensive report for your GP.

Your GP will discuss the recommendations with you and make any necessary adjustments to your medications.
